We provide proactive health programs that combine laboratory-facilitated blood panel testing, personalized results reporting, lifestyle protocol development, and health coaching services.
Nexis Health is not a medical practice, hospital, clinic, or healthcare provider. Our services are educational and coaching in nature.
Nexis Health is not a substitute for medical care. Our coaches are Health Coaches, not licensed physicians, nurses, or clinical practitioners. We do not:

Nexis Health facilitates access to laboratory blood panel testing through Quest Diagnostics walk-in locations. Lab orders are processed through an independent physician ordering service. You understand and agree that:
Includes a 60+ biomarker blood panel via Quest Diagnostics at wholesale lab cost, access to the Nexis patient portal, a personalized Nexis results page with plain-language wellness education, a personalized lifestyle protocol, progress tools, documents, and Nexis AI wellness education support. Core does not include health coaching calls or direct coach messaging.
Includes everything in Core, plus one health coach review call focused on your results, wellness protocol, priorities, and follow-up questions. Complete also includes coach messaging related to your results-and-protocol review. Any follow-up lab testing is recommended at coach and patient discretion and is not automatic unless explicitly stated in writing.
Includes everything in Complete, plus ongoing coaching access, recurring protocol support, a yearly full panel lab draw, continued patient portal access while membership is active, and priority access to future Nexis services. Coaching availability, cadence, and scheduling are subject to coach availability and reasonable-use program guidelines. Annual subscriptions renew automatically at the end of each billing period.

Health coaching is provided by Nexis Health coaches. Coaching is focused on lifestyle, nutrition, exercise, sleep, stress management, and supplementation guidance within the scope of health coaching practice.
Coaching calls included in Complete and Annual programs are conducted via video conferencing and scheduled through the Nexis patient portal or approved scheduling tools. Async coaching is available through the patient portal messaging system.
Coaching is not therapy, counseling, psychotherapy, or mental health treatment. If you are experiencing mental health concerns, please seek care from a qualified mental health professional.
By enrolling in a Nexis Health program, you understand and agree that Nexis may use secure technology tools to support program delivery, coaching quality, documentation, continuity, and patient support. These tools may include AI-assisted intake support, AI-assisted summaries, AI-assisted wellness protocol drafting, AI-assisted coach note drafting, appointment transcription, and coaching call recording.
AI-assisted tools are used to support Nexis coaches and operations. They do not replace human review, professional judgment, or your own relationship with a qualified healthcare provider. Coach notes, wellness protocols, and patient-facing guidance should be reviewed by a Nexis team member before being treated as final Nexis program material.
Coaching calls may be recorded or transcribed for note accuracy, quality assurance, coach continuity, and program support. Recording and transcription are not used to provide medical care, diagnosis, treatment, or emergency services.
If you do not want a coaching call recorded or transcribed, contact support@nexis.health before your appointment so Nexis can determine whether non-recorded coaching support is available for your program.
The patient portal at nexis.health/portal is a secure platform for eligible Nexis members to access their results, protocol, health tracking tools, documents, and Nexis AI wellness education support. Core includes portal access but does not include direct coach messaging or coaching calls. Complete includes one health coach review call and review-related coach messaging. Annual members receive ongoing access for the duration specified by their program.
You are responsible for maintaining the confidentiality of your portal login credentials. Notify us immediately at support@nexis.health if you believe your account has been compromised.
Nexis Health takes the privacy and security of your health information seriously. We maintain administrative, technical, and physical safeguards appropriate for a health coaching and wellness platform.
Nexis Health is not currently a medical practice, healthcare provider, health plan, healthcare clearinghouse, or HIPAA covered entity. We do not represent that our V1 health coaching platform is a HIPAA-covered medical service. If Nexis expands into services with additional regulatory requirements in the future, we will implement the compliance infrastructure and business associate agreements required for that model before offering those services.
Information you provide through the intake form, lab results, and coaching interactions is used solely for the purpose of delivering your Nexis Health program. We do not sell your personal or health information to third parties.

Third-party services used by Nexis Health to deliver programs include Quest Diagnostics for lab testing, Stripe for payment processing, Google Workspace for email, Beehiiv for email communications, approved scheduling and video tools, transcription or AI-assisted drafting tools, and trusted infrastructure providers for secure application hosting and data storage. Each service operates under its own privacy and security terms.

To the maximum extent permitted by applicable law, Nexis Proactive Health LLC, its coaches, employees, contractors, and affiliates shall not be liable for any direct, indirect, incidental, special, consequential, or punitive damages arising from your use of Nexis Health services, including but not limited to:
Our total liability to you for any claims arising from your use of Nexis Health services shall not exceed the amount you paid for your current program enrollment.
Nexis Health reserves the right to modify, update, or discontinue any aspect of its programs or services at any time. We will provide reasonable notice of material changes to enrolled patients via email.
These Terms of Service may be updated periodically. The current version will always be available at nexis.health/terms. Continued use of Nexis Health services after changes take effect constitutes acceptance of the updated terms.
These Terms of Service are governed by the laws of the State of Oklahoma, without regard to its conflict of law provisions. Any disputes arising from these terms or your use of Nexis Health services shall be resolved in the courts of Oklahoma County, Oklahoma.
